When scheduling appointments, please know that my cancellation policy is 48 hours notice. Appointments canceled or needing to be rescheduled without 48 hours notice are charged the full session rate. You agree to this policy upon scheduling an appointment. I do not bill insurance and am not contracted with any insurance companies. My rate is $225/90-minutes. Payment is made at time of service via cash, or credit/debit/HSA/FSA cards. While I do not bill insurance, I can provide an invoice that you can try to submit to your insurance company for reimbursement. Be sure to call your insurance company to figure out their reimbursement rates for out-of-network providers, policy, and process as they are all different. On the invoice I provide a massage procedure code (97124), units, and other pertinent information regarding the session. If your insurance company needs an ICD10 diagnosis code or prescription, you can get one from your midwife or medical doctor. Massage therapists do not have diagnostic authority and cannot generate diagnosis codes. Please be sure to give me the ICD10 code prior to requesting the invoices be created.
